How to Schedule an Intake

Contact a RASP case manager in your area.

Intakes can be conducted at the case manager’s office or at the client’s home and are usually scheduled within 7 days of initial contact.

Please bring the following items to the intake appointment

• Photo ID
  
• Proof of residency
  
• Proof of HIV Status
  
• Proof of income (if applicable)
  
• Proof of insurance (if applicable)
  
• Any medical records that may help us serve you better
